Take extreme care around the contacts on the connector in the middle. It has contacts on both sides of the raised section in the middle. Of course, if this is occurring only on one use cord/connector and the rest work fine, it's likely the cord itself. But if it's happening on multiple, try cleaning the port out.

So to connect to my external monitor it goes SL3 -> Surface Dock 2 -> External Monitor. The connection from dock to monitor is USB-C to HDMI Cable. My issue is that every time my device goes to sleep, shuts down or restarts; the external monitor is disconnected. I have to remove the cable and leave it for a minute then try plugging it back in.
